Job Description

As a Youth on Track Case Worker, you will play a vital role in empowering individuals by providing tailored support, resources, and guidance. You will work closely with clients to assess their needs, develop support plans, and connect them with essential services. Your ability to build strong relationships, advocate effectively, and navigate challenges will help drive positive outcomes for the people you support.

Key Responsibilities include

 

Reducing Police Contact - Reduce the young person's formal and informal contact with police at a higher level than those of a similar group of young offenders.

Addressing Criminogenic Risk Factors - Reduce the young person’s criminogenic risk factors.

Promoting Education and Employment - Increase the young person’s participation and attendance in education or employment.

Strengthening Family Interactions - Strengthen family interactions where appropriate or increase support from other positive role models.

Ensuring Continued Engagement - Engage young people and their families so they remain engaged in the scheme until meeting completion requirements.

What you’ll bring

  • Knowledge of the Criminal Justice System - Demonstrated knowledge of the Criminal Justice system.

  • Experience with Young People and Complex Needs - Demonstrated experience working with young people and adolescents with complex needs and their families.

  • Experience with Aboriginal Communities - Demonstrated experience working with young Aboriginal clients and their families.

  • Case Management Experience - Demonstrated experience working within a case management framework.

  • Collaboration with Service Networks - Demonstrated experience working with a vast network of services to ensure clear pathways to care and referral processes are maintained.

  • Goal Achievement and Risk Assessment - Demonstrated experience in working with young people to meet the goals of case plans, the young person’s needs, and risk assessments.

Eligibility/other requirements

  • This is an identified position for Aboriginal or Torres Strait Islander applicants.
  • Successful completion of a National Criminal Check.
  • Successful completion of an NSW Working with Children Check.
  • Valid New South Wales Driver’s Licence.
