Centacare New England North West is searching for a Case Worker to join our 'Keep on Track' Early Intervention Youth Program, you’ll work with young people and their families to tackle challenges before they grow. Job Description

Your role 

This role focuses on improving the mental health and wellbeing of young people, along with assisting with families through tailored case management, early support, and strong collaboration with local services. You will play a key role in identifying risk factors early and providing practical, strengths-based support to help young people thrive.

Key Responsibilities

As a Centacare Case Worker for the Keep on Track Program, your responsibilities shall include but is not limited to:

  • Case Planning & Risk Reduction
    Develop individualised case plans aimed at reducing risk factors for the young person.

  • Education & Employment Engagement
    Support and encourage the young person's consistent participation in education, training, or employment opportunities.

  • Family & Support Networks
    Strengthen family relationships where appropriate or help connect the young person with positive role models and support networks.

  • Goal-Oriented Support
    Ensure all case planning is informed by the initial assessment and aligned with the goals of the young person and their family.

  • Collaboration with Stakeholders
    Maintain strong working relationships with key stakeholders and partner organisations to ensure coordinated and effective service delivery.

  • Confidentiality & File Management
    Ensure the secure handling, storage, and management of all client records and personal information, with strict adherence to confidentiality requirements.

  • Ethical Practice
    Maintain a high standard of professional ethics and conduct at all times, particularly when working with children and young people.

  • Mobility & Flexibility
    Be willing to travel as needed to provide services, attend meetings, or participate in training.

  • Team Contribution
    Support other programs, staff, and clients as needed and carry out any other duties as requested by your supervisor.

Desired Skills and Experience

What you'll bring

  1. Demonstrated experience of work with young people with complex needs and their families.
  2. Demonstrated experience of working in a case management framework.
  3. Demonstrated experience of working with a vast network of services to ensure clear pathways to care and referral processes are maintained.
  4. Demonstrated experience in working with young people to meet the goals of case plans, the young person’s needs and risk assessments.

Eligibility/other requirements

  • Successful completion of a National Crime Check. 
  • Valid NSW Driver’s Licence.
